How is a mastectomy performed on men?

Gynecomastia surgery is carried out under general anaesthesia and takes approximately 1–2 hours. For minor procedures, local anaesthesia is used.

Surgically, the breast tissue around the areola is removed (mainly from the lower part) via an arc-shaped incision. The areola is reshaped to the correct size, leaving a small scar on the areola.

In the case of large breasts, it is necessary to remove not only the mammary gland but also some of the excess skin around the areola (by reducing the size of the areola).

We use absorbable sutures for greater comfort and to promote healing. After the operation, the patient is fitted with elastic compression garments, including an absorbent dressing, which form part of the post-operative care.

Gynecomastia in men and recovery after surgery

The treated areas may be sore or swollen, but this will subside within a few days.

Surgery for breast hyperplasia requires a 1–2-day hospital stay, which you will spend mainly in a comfortable bed; post-operative care then involves the need to wear an elastic bandage regularly for for approximately 2–4 weeks; this may be removed for necessary personal hygiene (the elastic bandage must be kept in place for 3–5 days).

Strenuous physical activity and sport are not recommended for approximately one month. A week of rest is recommended; after 14 days, you may resume your normal work and social routine, but overexertion is still not recommended for at least 6 weeks following the operation for breast hypertrophy.

After approximately 14 days, gentle breast massages using light pressure and the application of moisturising creams to the treated area are recommended.

Avoid sunbathing, sunbeds and saunas.

In rare cases, hypersensitivity of the areolae has been reported; these are temporary sensations that disappear after a few days.

The final result depends on the individual patient and their ability to follow post-operative care instructions, including their natural healing capacity and recovery process. The quality of the skin and subcutaneous fat also contributes to a good outcome.

Who is gynaecomastia surgery intended for?

Breast-firming exercises will not resolve gynaecomastia. The enlargement of the male mammary gland primarily occurs during puberty, when the breasts may become excessively enlarged.

It is reported to be associated with excessive sensitivity of the nipples and the surrounding areola, and is often accompanied by pain. Once pubertal growth has ceased, the symptoms usually subside in most cases.

In rare cases, however, the symptoms do not subside, and in such instances it is necessary to remove the excessively enlarged gland. This primarily affects young men aged around 30, and affects up to 30 per cent of the adult male population.

The unnatural appearance caused by male breast enlargement leads to significant psychological problems for many of them and makes their lives difficult.

Often, embarrassment and a lack of courage prevent them from undergoing a procedure such as surgery for an enlarged mammary gland, resulting in lifelong trauma and a lack of self-confidence.

How should I prepare for gynaecomastia surgery?

This is an invasive surgical procedure which requires the patient to be in good health. Before the operation, it is necessary to:

  • have a mammogram to confirm the diagnosis and rule out a tumour
  • to stop taking blood-thinning medication 14 days in advance
  • limit smoking

We do not recommend the procedure in cases of eczema, acne or inflammatory skin conditions.


How can I tell whether I have gynaecomastia or pseudogynaecomastia?

In pseudogynecomastia, there is no enlargement of the mammary glands – the enlargement is caused by being overweight or by a tumour.

Causes of true gynaecomastia include a drop in testosterone levels, hormonal fluctuations during adolescence, anabolic steroids used in sport, certain medicines or addictive substances, excessive alcohol consumption, liver, thyroid or adrenal gland disorders, and genetic predisposition.
