About the procedure

Thanks to modern venous surgery, it is possible to offer patients comprehensive care for venous system diseases – varicose vein surgery. We use minimally invasive methods and treatment is performed on an outpatient basis, without the need for anesthesia. We utilize the most modern and highly innovative minimally invasive methods (thermal – laser/radiofrequency ablation, non-thermal – venous glue, mechano-chemical ablation).

Our philosophy is based on the continuous education of our specialists, so all our procedures comply with current recommendations from international professional societies. The goal is not only the therapeutic effect but also the aesthetic result of the procedure.

Varicose vein surgery - when to undergo?

Varicose veins (varices) are one of the manifestations of chronic venous insufficiency (chronic vein insufficiency – CVI), specifically the second stage of this condition. The most common manifestations of chronic venous insufficiency include pain, swelling, and visible varicose veins (varices), which in the initial phase may not limit the patient in any way; however, as the disease progresses, chronic venous insufficiency can manifest itself, for example, through the development of inflammation or leg ulcers. In rare cases, the disease can even result in pulmonary embolism.

Treatment is primarily indicated for symptomatic patients who suffer from swelling, cramps, pain, and a feeling of heavy legs. Targeted treatment will eliminate symptoms and prevent the progression of the disease to a more severe stage. We also offer treatment to patients for whom visible varices represent an aesthetic problem and who do not show signs of CVI.

Types of procedures we perform:

Venous surgery includes all minimally invasive methods for the treatment of varicose veins.

 

Minimally invasive treatment methods

  • the procedure is performed on an outpatient basis and lasts no longer than 30-45 minutes
  • no preoperative examinations, only partial anesthesia, no need for hospitalization
  • during the procedure, the affected vein is closed by thermal or chemical means, the procedure is completely safe due to ultrasound control from several skin punctures, a skin incision is not necessary
  • the removal of the affected vein does not occur in the same way as with classical invasive methods, the procedure causes the closure of the treated vein, which subsequently turns into a scar and gradually absorbs
  • compared to classical invasive treatment, the main advantage of minimally invasive methods is low postoperative pain, and the patient can immediately return to their daily activities including work

According to the mechanism of closure, we divide minimally invasive methods as follows:

Thermal minimally invasive methods (destruction of the vein by heat) – the procedure is performed under local, so-called tumescent anesthesia – the treated vein is surrounded by a solution with anesthetic, the aim is to prevent damage to surrounding tissues due to heat.

  1. Laser ablation (EVLT – endovenous laser treatment) – a laser fiber is inserted into the affected vein, which destroys the inner wall of the vein due to thermal energy, resulting in its closure

Non-thermal minimally invasive methods (destruction of the vein without heat application), without the need for anesthesia

 

  1. Venous glue – venous glue (cyanoacrylate) is applied to the affected vein.

If the lateral varicose veins are larger, all minimally invasive methods can be supplemented with so-called phlebectomy/sclerotherapy; sclerotherapy can be performed during the procedure or with a delay of 6 months.

At the patient’s request, we can supplement the procedure with analgesosedation. This involves controlled pain and anxiety relief in the presence of an anesthesiologist, but it is not general anesthesia.

 

Sclerotherapy (foam sclerotherapy)

This is a method where a special foamed substance (foam sclerotherapy) is injected into smaller varicose veins that are not suitable for surgical treatment. The action of this substance leads to a non-infectious chemical inflammation in the wall of the vein. The affected vein subsequently closes and the venous pattern disappears from the skin. This method is subject to strict indication. First, it is necessary to address the main cause of the disease: most often it is the so-called reflux into the main trunk vein. Sclerotherapy is usually performed as an adjunctive treatment after resolving the cause of varicose veins. As a standalone treatment, it is performed only when the underlying cause of the disease has already been removed.

 

Conventional surgical treatment

  • requires general anesthesia and hospitalization, the operation is performed on the day of admission, the patient is discharged the following day
  • complete preoperative examination
  • the principle of the procedure is “stripping” of the affected vein from several skin incisions
  • the procedure is usually supplemented with so-called phlebectomy – larger varicose veins are removed from several smaller incisions
  • the main disadvantage of traditional surgical treatment of varicose veins is postoperative pain, long recovery (4-6 weeks), and the risk of postoperative wound complications, which we do not have to worry about with minimally invasive methods
